I had the same equipment as a guy who attacked me, and all of my stats were higher, but every single one of my attacks missed. What causes this?

Maybe because you had low (or no) stamina? Or because your speed or strength are low.

Its your stamina. Even if you hav higher stats than the guy your fighting, you'll keep missing because of stamina being low.
